Case Nurse Manager null

Roles and responsibilities:

1. Review all new admissions orders.
2. Follow up with field RNs to receive SOC reports for all new admissions. 
3. Send reports on all new admissions to office staff to ensure timely staffing.
4. Request authorizations for Kaiser and PPO patients from Billing Department.
5. Provide support to field clinicians as needed.
6. Keep patient census current and up to date. 
7. Process any orders for patients on service such as Labs, DMEs, supplies, etc. 
8. Keep close communication with patients to explain services and answer any questions they may have.
9. Document all reports received from field clinicians and take action as necessary.
10. Request orders from MD as needed/'Report lab results to MD.
11. Participate in the coordination of home health services appropriately reporting the identified needs of patients to the interdisciplinary team which includes PT/OT/MSW/ST/HHA/RN/LVN.
12. Track high acuity patients to ensure visits are being done timely and correctly.
13. Collaborate with Staffing team to relay staffing needs for patient and ensure patients have been adequately staffed.
14. Review patient’s chart and track progress, determine patient needs throughout the duration of patient being on service.
15. Keep patients charts up to date with current tracking and documentation of findings
16. Ensure patients get timely services.
a. Track patients coming due for recertification of services, ensure staffing is made aware to schedule RN for recert, get recert report from RN in a timely manner to ensure continuity of care without interruptions.
b. Track patients returning from the hospital – review new discharge orders, document and change in patient condition, communicate with staffing to have RN conduct ROC visit in a timely manner, receive ROC report from field RN.
c. Discharge patients in a timely fashion, ensure chart completeness at the end of services.
